Health Facilitation Service (Services for People who have a Learning Disability)

Health Facilitation Service: Nurse-led service, working with adults who have a learning disability to enable them to access and receive equitable healthcare from services.

We are part of ‘Services for People who have a Learning Disability’ (SPLD) who provide specialist healthcare services to people who have a learning disability and are 18 years and over, living within Bedfordshire and Luton.

The service is provided for people who have:
• Challenging or disturbed behaviour
• Degenerative neurological disorders which occurred during their developmental period and led to learning disability
• Epilepsy which cannot be managed by general neurological services
• Mental illness and other mental disorders such as autism
• Sensory deficits which cannot be managed by mainstream services

Intensive Support Team (IST) (Services for People who have a Learning Disability)

Intensive Support Team (IST): Provides a 24-hour crisis service as well as multi-disciplinary (Nursing, Psychiatry, Psychology, Occupational Therapy, Speech and Language Therapy community based services to people who have a learning disability and are 18 years and over, living within Bedfordshire and Luton, who are experiencing a mental health or challenging behaviour crisis requiring urgent assessment and treatment. Where possible service is provided in the individuals own home, with the intention of minimising the need for admission. Where the level of risk is such that hospital admission is required, the IST can assist in minimising inpatient stays by facilitating early discharge and supporting people back into the community. 24 hour Crisis Service is available on 01234 310 538 where a nurse is available and if required will visit the person within 4 hours.

Occupational Therapy (Services for People who have a Learning Disability)

Occupational Therapy helps patients identify the occupational elements that contribute to the difficulties they have carrying out meaningful activites and provides advice, support and patient-centred intervention to achieve identified occupational goals, with the ultimate aim being the improvement of physical and/or mental health and sense of wellbeing.

Physiotherapy provides access for patients to generic physiotherapy services or to provide specialist intervention as required or indicated from the inital assessment, as well as promoting activites which complement the patients overall physical management programme and their health.

Sensory Impairment Service (Services for People who have a Learning Disability)

Sensory Impairment Service: Supports patients to gain access to appropriate sight and hearing assessments, and develop the use of their senses, as well as providing Primary Ear Care and health promotion.

Specialist Medical Department (Services for People who have a Learning Disability)

Specialist Medical Department: provides a consult - led service to patients through specialist psychiatrists working within the three pathways of the service, mental health with challenging behaviour, complex physical needs and forensic services.

Speech and Language Therapy (Services for People who have a Learning Disability)

Speech and Language Therapy: Supports patients through assessing the level of language and communicative functioning and determines the potential to maximise these skills through a variety of communication. They also act as interpreters for service users with communication difficulties. The services provided by those listed above can be accessed by both inpatients and outpatients depending on individual need.
