Preparing for Adulthood
All young people and their families face big changes together when they leave school and need to make plans for their future. The purpose of person centred planning is to help and support young people to achieve their personal goals.
National guidance suggests that what young people want most is to 'get a life' and be part of their local community. There are a number of areas to think about that will help to achieve this, these are:
- Pathways into employment
- Education choices post 16
- Planning for good health
- Housing pathway
- Developing friends, relationships and community
